Most of the classes are on Saturday and Sunday and include lectures, demonstrations, and on-hands training observed by the instructor.  You will receive classroom instructions of various techniques and the correct time in the therapy to use them.  The instructor for most of the classes will do multiple demonstrations on clients and any student that volunteers.  During these sessions the instructor will lecture why certain techniques where used at different times and what is going on when doing the demonstrations.  The student will also be observed doing on-hand work under the instructor's supervision one at a time, as other students watch.  After which there will be a question and answer period.  The other students will be allowed to make comments what they felt was done the best and suggestions what they feel could be done differently from their observations.  The person demonstrated on will also be able to give helpful feedback. The class sizes are limited to ensure maximum individual attention from the instructor.

All classes are taught by instructors with many years of experience and training in their field, and are highly qualified.

Only students that attend Indiana State Approved schools can qualify to become Certified to practice in Indiana.  Indiana require 500 classroom hours of teaching.  Schools in other states can not practice in Indiana.  However, anyone taking classes from Indiana schools that are Indiana approved, like this institute, can practice anywhere in the country.  Hypnotherapists that are not trained by Indiana approved schools and certified by Indiana can not practice in Indiana.

Midwest Training Institute of Hypnosis will accept credits from other accredited schools approved by the Indiana Hypnotist Committee.  Please contact Midwest for assistance in seeking approval by the Committee.

CURRICULUM

CLASS #101:

HYPNOSIS/HYPNOTHERAPY
(ALSO OFFERED AS A CORRESPONDENCE COURSE) 
Saturday and Sunday    8:30 AM to 5:30 PM

The History of Hypnosis will be taught. Different depths of hypnosis will be taught and how to apply this to motivate clients to Stop Smoking, Control Weight, Control Stress, and help to make other changes The correspondence student is sent instructions and the materials needed for this course. This includes a manual, as well as tapes of class teachings, demonstrations of various inductions. Student will have on-hands training. 

With a passing score and completed work students will receive a certificate of completion. No previous training required.

TUITION: $304.00 - A $100.00 pre-registration fee is required to ensure your place. Limited seating (After paying the pre-registration fee, payments can be arranged for the remainder of the tuition. This must be arranged before the first day of class) Class includes manual of instructions

CLASS  #102:

REGRESSION THERAPY
(ALSO OFFERED AS A CORRESPONDENCE COURSE)
Three Sat. and Sun. Every other Weekend
First and last weekend hours are 8:30 AM to 5:30 PM

  

Learn how to apply hypnosis to therapy, to regress clients with different techniques, back to the incidents that caused their problem. Demonstrations of applying different techniques to work out and resolve the emotions connected to their problem in a quick and gentle way.

The instructor will demonstrate regression therapy on volunteers. Audiotapes of actual sessions with clients for various problems are included for homework. (When correspondence students have all materials read and all homework has been returned, the student must notify the instructor in writing that they have completed the work).

Only when the instructor feels the student is ready can the student then demonstrate that they can do regression therapy. The student is not to do regressions on anyone until the instructor determines that they are qualified to do so. Questions that will be on the final test will be covered in the course. The student may call the Institute whenever they have questions or need information during their correspondence course. With a passing score students will receive a certificate of completion.

Previous hypnosis training from any school or conference is a prerequisite.

TUITION: $912.00 - A $300.00 pre-registration fee is required to ensure your place. (Limited seating). After paying the pre-registration fee, payments can be arranged for the remainder of the tuition. This must be arranged before the first day of class. Includes manual of instructions that will be e-mailed to you to print out after the pre-registration fee has been received.

THREE WEEKEND CLASS: 6 days Total of 48 hours.      

 

CLASS #103:

ANALYTICAL HYPNOTHERAPY USING IDEOMOTOR SIGNALS
(ALSO TAUGHT AS A CORRESPONDENCE COURSE)
Fri., Sat. and Sun.  

A series of yes and no questions are asked, with the client answering with yes and no finger signals that are given by their subconscious mind. This type of therapy leads to events that are locked into the subconscious and not revealed to the conscious mind. Therapy can still be completed. This would have not been possible with the usual type of therapy where the client must know what they have regressed to and verbally tell the therapist what is happening. This technique is private since the client does not disclose verbally the details of the regressions. Since the majority of the time the client is unaware of the details of the incidents there is no trauma. Analytical hypnotherapy will enable you to get results you may not have been able to accomplish otherwise in about one-half the time. Hands-on training.

Previous hypnosis training is not a prerequisite but helpful.

Course material includes the manual of instructions which will be e-mailed to you to print out after the pre-registration fee has been received. Previous training how to hypnotize is necessary.

TUITION: $456.00 (A $100.00 pre-registration fee is required to ensure your place, limited seating) Payments after the initial $100.00 pre-registration fee can be arranged. This must be done no latter than the day before the first day of class.

WEEKEND CLASS:  3 days 8:30 am to 5:30 PM Total of 24 hours.

 

CLASS #104:

REGRESSION THERAPY USING KINESIOLOGY (MUSCLE TESTING)
Fri., Sat. and Sun.  

This class is identical to Class #103. The difference is that it is not done with the client hypnotized. The regression therapy is done with the yes and no muscle testing which is also an Ideomoter signal called Kinesiology. It is a technique frequently used by Chiropractors. The client's arm is extended out and after asking a yes and no question the therapist says to the client, “Resist” and then gently pushes down on their wrist with two fingers. If the answer from their subconscious is no the arm will drop down. If it is yes it remains stable. When the source of the problem is found then the non-hypnotic technique is taught how to resolve the feelings and emotions. This is a good technique to use when

Individuals may have a problem with hypnosis. It is again a gentle and quick form of therapy. The client is not traumatized since they do not consciously know what the subconscious has regressed to. Previous training is not necessary.

CLASS #105:  HYPNOTHERAPY FOR CHILDREN

                                                    

Therapy using hypnosis for children is different then what is done for adults. Techniques vary depending on the age of the child. You will be taught how to have the parent approach the child to prepare them properly for them to be receptive to the therapy. Manual included

CLASS #106:

HYPNO-ANESTHESIA, GLOVE ANESTHESIA, PAIN CONTROL 
IMPROVED HEALTH/IMAGERY(ALSO TAUGHT AS A CORRESPONDENCE COURSE)
Sat. and Sun.

You will be taught how to achieve the depth of hypnosis for anesthesia in operations, painless dental work, and how to teach expectant mothers painless childbirth. You will learn how to program the client to be able to numb a hand and transfer the numbness to any part of their body. After you teach them this technique, they will be able to do this without having to put themselves into a state of hypnosis. This is especially great for dental work, and childbirth.

You will learn how to teach clients to eliminate or lesson chronic pain, improve health and achieve healing more rapidly with the power of their own mind. This is also to help the client respond more effectively to the doctor’s treatment. Videotapes of operations with hypnosis will be used and the use of hypnosis for pain control and rapid healing for patients will be shown. Hands-on training.

A test will be given at the end of this course. The student must also be able to demonstrate the procedures taught to get a passing grade. Previous knowledge how to hypnotize is helpful but not necessary.

Course materials includes handouts.  Audiotapes of different techniques and sessions with clients for improved health and teaching them pain control and imagery can be purchased.

CLASS #107:  READING BODY LANGUAGE

We often can tell, without realizing it, what people are really thinking by their body language. In this class you will be taught to recognize all the subtle body language that you are not familiar with. This enables you to be more knowledgeable in dealing with people in a more effective way.

CLASS #108:  INFLUENCING WITH INTEGRITY (NON-VERBAL APPROACH)

In this class you will learn the skills to influence non-verbally your communication skills to achieve the outcome you wish to achieve. This is a powerful skill to learn and must be used with integrity.

CLASS #109:  INTRODUCTION TO PSYCHOLOGY

The fundamental principles of psychology including but not limited to research methodology, perception, development, motivation, consciousness, learning, thinking, stress management, and social relationships.

CLASS #110:  CHILD PSYCHOLOGY

An introduction to the study of the developmental stages that children go through. This course could easily have been entitled the Developmental stages of Childhood. 

This course is an introduction to Humanities. Child Psychology encompasses quite a large variety of subjects, everything from major developmental theories, cultural research, theoretical perspectives, genetics, motor development, psychosocial development, and childhood diseases, to adolescents.

CLASS #111:

EMOTIONAL FREEDOM TECHNIQUES (EFT)
AS AN ADJUNCT WITH HYPNOSIS

Emotional Freedom Techniques™ (EFT) is like acupuncture without the needles. It is a self-help tool which combines fingertip tapping of key acupressure points with focused thought. EFT has helped thousands all over the world eliminate cravings, anxieties, phobias, traumatic memories, physical pain, and negative emotions. The results are often described as amazing.

Individuals completing these intensive practical classes will develop the ability to use Emotional Freedom Techniques (EFT) as an adjunct with Hypnosis, Medical, Dental, and Chiropractics techniques effectively and with confidence. Each student will learn through lectures, demonstrations and visual aids the applications of the many methods of (EFT) Students will participate both as a participant and practitioner.

CLASS #112:  SMOKING CESSATION CLASS (how to do groups and Corporations   

                             and more)

You will learn how to set up and conduct group "Stop Smoking Workshops". This class will cover reviewing hypnotic inductions and techniques, addiction or compulsion, ridding the body of nicotine, determining the real reason(s) for wanting to abstain. In addition the importance of water (flushing out the nicotine), vitamins for nerves and stress. With this technique there should be no weight gain after stop smoking. How to Market your "Smoking Cessation Program", to individuals, institutions and corporations. Who to contact, pricing your program, and setting up a follow-up system.  There will be handouts.

Previous hypnosis training is required.

CLASS #113:  THE BUSINESS SIDE OF HYPNOSIS

This is a very important class if you intend to go into business for yourself. Students have as much as tripled their business in one or two months after this class. You will learn: How to form your practice (Corporation, Sub Chapter S, LLC or Proprietorship), Where to conduct your business, Business Basics (Insurance, Licensing, appointments, etc.), Paperwork and systems to make your business flow smoothly, Promoting your business (Marketing, Advertising, Networking, Prospecting), Filling your schedule, Client Follow-up

CLASS #114:  SPORTS HYPNOSIS

Hypnosis is a helpful tool to improve any type of sports performance. You will learn how to help the sports client with imagery, and giving them a key word to tape into achieving the best game they have ever had. If a client has a specific problem in their chosen sport Regression Therapy can get to the cause and resolve the problem Arnold Palmer went to a Hypnotist early in his career and greatly improved his game of golf to win many competitions.  There will be handouts.

Previous hypnosis is a requirement.

#115: Advance Instruction

Demonstrations and Lab time that will give the student the necessary hands- on skills. (for any class from #101 to #106) 

This class gives the student ample opportunity to get the practice needed to be confident that they are doing the procedures correctly.

 

WEEKEND CLASSES     Saturdays and Sundays  9:00 AM to 5:00 PM

                        For a total of 240 hours.  Check with Institute for dates.

                       

TUITION:  $150.00 for each weekend..  All practice days are scheduled on weekends and are on going for all PRACTICE CLASSES AT THE SAME TIME.  You can sign up for the weekends you want till you get the hours you need done.

A $25.00 pre-registration fee is required to ensure your place.  Limited seating.

Thank you so much for my past session.  I am amazed how good I feel compared to the last few dismal months of depression.  It is like day and night difference!   I have done publicity work for you by telling several friends about it.

My best friend suffers from gout, arthritis, and acid reflex, her daughter is on disability for fibromyalgia and her grandson has MS (found out when he was 17 and now is 21)..whom also just went on disability, and has constant pain problems as now parts of his body are going numb.   I was wondering if you could help with their pain.

I have seen you only 3 times for issues dealing with my uncontrollable snacking and mindless over eating.

I come from a history of molestation as a small child and have been having a hard time dealing with these emotions in my mid adult years. Obviously I was using food as a way to comfort myself and yet, I didn't know how to stop myself.

I was also feeling as if I was always filled with anxiety, I was a restless person, and had trouble sleeping.  After my third visit with you, our chaplain made a comment to me that said volumes.  He told me I looked "rested and calm".  No one has ever said that to me.  He also had no idea that I had started sessions with you.

I brought my friend to be a volunteer for you to demonstrate on him, in your class I was taking in hypnosis. The Doctors gave him three months to live. He had a liver disease. His complexion was gray and his ear lobes and fingers were black. He walked with a cane and continually belched air. After you were done with the session on him his skin color was normal, ear lobes and fingers were now gray instead of black and there was no more belching. He handed me his cane and said he no longer needed it. In a couple of weeks my friend was able to eat anything for the first time in a year, and he felt stronger with each day. In three months the doctors said that there was no longer a problem with his liver. They had not done anything different in his treatment. There has been no reoccurrence since then in the past years.

I served as a Green Beret Sergeant on a Special Forces A team in Vietnam in 1969. I saw and participated in a great deal of heavy combat at the Cambodian border. We were used as bait to lure main force North Vietnamese infantry units into combat so that we might attack them with air artillery strikes. We took no prisoners. In the end we went through seventy-one straight days of fighting. I saw several of my close buddies blown up near me.

Upon arrival in the U.S., I immediately sought psychiatric help from the VA. I heavily abused alcohol and drugs. It cost me my marriage. People, even those who knew me were deathly afraid of me.

For the next 25 years I had continuous nightmares, flashbacks, hallucinations. I received continuous psychiatric treatment from the VA, including six hospitalizations for PTSD. Nothing really helped.

In April of 1995 I was at the end of my rope. On a chance I saw Dr. Zukausky in the newspaper. I made an appointment. It changed my life.

After the first session my mother asked what had happened. She noticed a drastic change in my attitude and demeanor. When I told her about my experience with hypnotherapy, she wept. She had her son back.

By the fifth session I had ceased to have nightmares, and the guilt I had felt for a quarter century was gone. I have never had flash backs or nightmares. I have learned to release my anger rather than internalize it. The violent feelings I would experience are gone. All because of hypnotherapy and this very special lady.

I first went to Gisella Zukausky for hypnotherapy with a referral from the Family Practice Center Of The Fort Wayne Medical Education Program on Feb. 5, 1999.

I had many problems that I had been in therapy for almost twenty years. I was diagnosed as Schizophrenic with Manic Depression, and heavily medicated. I had many fears, obsessive compulsive, mental distractions, grandiose thinking, to name only a few. Hearing voices was also a major problem for me for many years.

After seeing Gisella Zukausky 20 times that summer I have achieved the following results:

Fear 7 session, almost totally gone Heavy heart 2 sessions, these feelings are totally gone. Sour sinking feelings Depression 2 sessions seems to be totally gone. Obsessive behavior 3 sessions, this has lessened but probably need some more work. Insecurity 5 sessions, mostly gone but still seems to need a little more work. Hearing Voices 2 sessions, now gone. At some session we worked on more than one problem. to get more help done. Mental Distraction, Lack of Confidence and Self-Esteem, Not Feeling Cheerful, Grandiose Thinking/Because of Delusions. Since these sessions I have gotten a job, and am now in a serious relationship. I have plans to go back to college to finish my degree. I had dropped out 20 years ago. In the fall of 1999 I fell asleep while driving my car and caused the death of someone. It was very difficult for me to deal with this. I had been receiving therapy from someone for this. When I called Dr. Zukausky she offered to work on me for free since I did not have the money at the time, to resolve these feelings so they would not trouble me and cause me more difficulty. After three sessions I felt I was able to put that incident to rest and felt I now was free from the feelings this had caused me.

Hello Gisella,                   ADDITIONAL HELP FOR OTHER PROBLEMS

Just to let you know that Rick and I are doing wonderfully now. I feel much better about myself and the sessions helped in ways I didn't plan on. I have digestive problems from having gastroparesis (Paralyzed Stomach), and have stomach spasms regularly, especially at night. Also some undiagnosed "twitching: throughout my limbs. After the three sessions with you, the spasms and twitching are probably 75% or more relieved even though we never worked on those problems. I can sleep I I never even told you about that problem. So thank you for that bonus.

This was something that was so helpful after years of seeing psychologists, I felt better in just 3 visits for the problem I came in for.

I am studying for a psychology degree currently (although in the early stages). I really want to be able to do this for others as years of counseling and therapy can be accomplished in only a few sessions with Clinical Hypnosis.

POST-TRAUMATIC STRESS DISORDER

I am a 52-year-old male that had tried to deal with PTSD for nearly 33 years. After 30 plus years of private and VA therapy I was lucky enough to encounter you, Dr. Zukausky. July 4, 1972 off the coast of Vietnam I was injured aboard the U.S.S. Newport News and as a result of those injuries, I was deaf and blind. The US Navy doctors rebuilt my ears and restored my sight. I didn't feel very lucky; in fact I was a very angry man. I unconsciously obscured that anger from myself, but was not as successful in keeping it from upsetting those around me. I, like many of us, showed largely that anger in what we call "Road Rage". I was consistently judgmental of everyone else on my road. After two sessions with you I was no longer judgmental or criticizing other drivers. I was observing them instead, "Oh, she's on her cell phone." Not "Get the _____ off the phone!" The stress is gone and I feel like I've added years to my life. Life is less of a burden now. My relationships with others and my wife are much happier now.

I had a very bad problem with cocaine & alcohol. This problem lasted for about 14 years. The drinking started at the age of 13. I had my first drunk at 14. I have been in numerous "In Patient" programs. I did have some clean time in between. In fact, I did have 2 years plus at one time. I started drinking (first) & then using. I stayed out for over 2 years. It was a true two years of Hell! Things were so bad, I even tried to end it a couple of times. It is a "True Miracle" that I'm even here. Then I met Gisella Zukausky & started weekly sessions. Within 8 sessions the obsession to drink or use was GONE. I was even able to stop chewing, a habit that I had for over 15 years. Dear Gisella:                             PERMANENT WEIGHT CONTROL

I just wanted to let you know how well the hypnosis for weight loss is working for me.  After my second session with you, that evening my husband and I went to eat at a Chinese restaurant.  As you know, at most Chinese restaurants the portions are quite generous.  As I explained to you in our first session, as rule I don't drink anything with my meal. This time I followed your suggestion of drinking water between bites.  I also had some hot tea to drink along with my meal and alternated tea and water during my meal.  This did slow down my eating pace and allowed me to enjoy my meal and eat until my stomach told my brain I was full.  Here is the most exciting thing - the waitress brought us boxes to take home the leftover.  When I looked at what was left over without really truly "thinking" about it I knew that what I had left would be enough to satisfy me for two lunches - so I used 2 boxes to take my meal home. 

Two days later we had "chocolate" day where I work at Kohl's.  We had all kinds of chocolate candies, most of which are my "favorites".   Without any effort whatsoever, I totally ignored the candy.   I had no interest whatsoever.  Prior to seeing you I would have been in that break room a number of times grabbing 2-3 pieces at a time, not even being aware of how much chocolate I had consumed during the day.  I am so pleased with what is happening. I am telling many people about this program.  I noticed today on my 3rd session I was very much anticipating being hypnotized.  I feel wonderful after the session ends.

WHAT IS CLINICAL AND MEDICAL HYPNOSIS?

The Medical Licensing board in Indiana governs Hypnosis. They require that we give a new client in writing our training and qualifications. Anyone using Hypnosis in Indiana must be Indiana State Licensed. Only Physicians, RN's, Dentists, Licensed Psychologists, Psychiatrists, and Social Workers, are exempt from taking the test to be licensed. The amount of training in Hypnosis may vary from two days if the hypnotist only gives Post Hypnotic Suggestion. It takes a great deal more training if the Hypnotist does Regression Therapy to get to what is causing the problem and resolving the feelings and emotions, or Medical Hypnosis.

Clients that come in for therapy often wonder what hypnosis will feel like. They are always surprised the first time that it feels no different than sitting with their eyes closed. Unless a convincer test is done on them to show that they are in a state of sub-consciousness (hypnosis) they would think nothing happened. Anyone who has ever been hypnotized will tell you that you are fully aware, awake, and in control of yourself. It is all self-hypnosis. The individual listens to what the hypnotist says and then allows the relaxation to take place physically and mentally. No one can be hypnotized unless they want to be or kept in a state of hypnosis if they choose to bring themselves out of that relaxed, hypnotic state. It is somewhat like listening to instructions to relax and allowing your subconscious mind to let that relaxed hypnotic state happen. Therefore, the client is actually hypnotizing (mentally relaxing) himself or herself.

Whenever we are in a daydream state or meditating  and are making no conscious effort to think of anything in particular, it is what we call hypnosis. The brain waves are at the same level when we are meditating or daydreaming. We are then in our subconscious mind, not the conscious thinking part of our mind. That is when we have our most creative thoughts. When we are not thinking we are in our subconscious mind, which is like a computer. That is where all our experiences and what we have learned, are stored. A very important point is that all of our emotions and feelings are stored in our subconscious mind. Our conscious mind is where we think and reason. We are using only 2% to 7% of our conscious thinking part of our mind. We don't know consciously what all the incidents in our past are that are responsible for a problem.

If a client is hypnotized (mentally relaxed) they are in a highly suggestible state of mind.   However, they have a free will and will not do or accept any suggestions of anything that is against their principles and beliefs. Only suggestions that they feel are acceptable and in their best interest will be accepted. That is why just doing therapy with hypnosis were the client is given only Post-Hypnotic Suggestions to hopefully cause a change and to improve, are only about 15% successful. If the client finds a suggestion that is phrased in a negative way or is unacceptable or doesn't like the suggestion they will reject it. They will also reject a suggestion to change if they are not willing to change.

The client is always in control and can reject any suggestion if they find it unacceptable. The results are seldom permanent with only suggestions to change, since what caused the problem hasn't been resolved and removed. For example: When an individual tries to stop smoking with the Patch, Nicoret gum, acupuncture, etc. or by only using Post Hypnotic Suggestions from a Hypnotist, this does not get to the source of what has caused the problem to develop. The success rate is less than 15%. Of those 15% when there is enough stress they will start up smoking again or in some cases substitute another habit such as now over eating to pacify them selves.

When Hypnosis is used Clinically to get resolved what has caused the Smoking the success rate is about 98% without any weight gain. This is because the emotions that were the trigger are no longer there to be pacified. The sessions are done one on one since it is therapy under hypnosis.

Dr. Zukausky has a technique she has developed to eliminate the feelings and emotions that have caused the smoking for Corporate Stop Smoking group sessions that has achieved the highest rate of success compared to the method used by other Hypnotherapist doing group sessions with only using suggestions to stop smoking.

We react to whatever is in our subconscious mind. Some times it conflicts with our conscious thinking. That is why we don’t always do what is in our best interest, but react to our feelings and emotions instead. For example this is why a person knows they should Stop Smoking but has so much difficulty being able to. They are reacting to the feelings and emotions in their subconscious mind that need to be pacified. 

By law any Hypnotherapist is to give in writing their training and credentials from the school institute that trained them.

It is important for the client to know what the kind of training of the Hypnotherapist is. Did they have training in "Regression Therapy"? This kind of therapy training means they can find out what is motivating the behavior in their past and then eliminate and resolve it. Then the client no longer has the need for the problem. The results are also permanent. The client should find out how effective the hypnotherapist is when choosing one by asking for referrals and testimonials from others. Any hypnotherapist should be willing to give this information.

GENERAL PROVISIONS:  This Agreement Must Be Signed 6 Days Before Beginning Of Class.

 I understand that I will be entitled to the privileges and bound by the conditions below:

Cancellation: I understand that I may cancel this Enrollment Agreement in writing, within six business days of signing the agreement, and will be entitled to a full refund of any money paid.

Financial Obligations: I understand that I am personally responsible for all tuition, fees and other charges arising from and during my enrollment at Midwest Training Institute of Hypnosis.  

I understand that it is my personal obligation to pay all tuition, fees, and other charges when due. If I do not pay the full amount of any scheduled payment when that payment is  due, I may not be permitted to continue my studies. If I am not able to continue my studies, I understand that I am responsible for any outstanding tuition; fees or other charges due in accordance with the Institute’s refund policy. I understand that my academic transcript   will not be released to me or to any other individual requesting my transcript if there is  a balance due to Midwest Training Institute of Hypnosis. In addition, if I choose to re-enroll at Midwest Training Institute of Hypnosis, l must satisfy any outstanding tuition, fees, or other charges prior to my re-enrollment.

Instruction: I understand that I will receive instruction in English, in lecture and laboratory formats. Instructions will be delivered by qualified instructor Midwest Training Institute of Hypnosis.

Renewal of Enrollment Agreement: I understand and agree that although this agreement is executed for a period of only one (1) class, my enrollment at Midwest Training Institute of Hypnosis for, subsequent class's shall constitute a renewal of the terms of this agreement except for the tuition charge and fees, which may be subject to change with at least one class notice to students. 

ACKNOWLEDGEMENTS:  I understand that Midwest Training Institute of Hypnosis reserves the right to make changes in program content, materials, or schedules as it deems necessary.  The Institute further reserves the right to discontinue my training for unsatisfactory progress or attendance, non-payment of tuition or fees, or failure to comply with the Institutes policies and procedures.

570 IAC 1-8-6.5 Resident institutions, refunds

     Sec. 6.5

     (a) The post secondary proprietary educational institution shall pay a refund to the

           student in the amount calculated under the refund policy specified in this section or as otherwise

           approved by the commission.  The institution must make the proper refund no later than

           thirty-one (31) days of the student’s request for cancellation or withdrawal.

      (b) The following refund policy applies to each resident post secondary proprietary educational

            institution, except as noted in section 4.5 of this rule:

 (1) A student is entitled to a full refund if one (1) or more of the following criteria are met:

                           The student cancels the enrollment agreement or enrollment application within six (6) business days after signing.

                          The student does not meet the post secondary proprietary educational institution’s minimum admission                                   requirements.

                 

                           The student’s enrollment was procured as a result of a misrepresentation in the written materials utilized by                             the post secondary proprietary educational institution.

               

                            If the student has not visited the post secondary educational institution prior to enrollment, and, upon touring the                           institution or attending the regularly scheduled orientation/classes, the student withdrew from the                             program within three (3) days.

(2) A student withdrawing from an instructional program, after starting the instructional program

     at a post secondary proprietary institution and attending one (1) week or less, is entitled to

     a refund of ninety percent (90%) of the cost of the financial obligation, less an

     application/enrollment fee of ten percent (10%) of the total tuition, not to exceed one hundred

     dollars ($100).

(3) A student withdrawing from an instructional program, after attending more than one (1) week

     but equal to or less than twenty-five percent (25%) of the percent (75%) of the cost of the

     financial obligation, less an application/enrollment fee of ten percent (10%) of the total tuition,

     not to exceed one hundred dollars ($100).

(4) A student withdrawing from an instructional program, after attending more than twenty-five

      percent (25%) but equal to or less than fifty percent (50%)  of the duration of the instructional

      program, is entitled to a refund of fifty percent (50%) of the cost of the financial

      obligation, less an application/enrollment fee of ten percent (10%) of the total tuition, not to

      exceed one hundred     dollars ($100).

 

(5) A student withdrawing from an instructional program, after attending more than fifty percent (50%)

      but equal to

      or less than sixty percent (60%) of the duration of the instructional program, is entitled to a refund of

      forty percent (40%) of the cost of the financial obligation, less an application/enrollment fee of ten

      percent (10%) of the total tuition, not to exceed one hundred dollars ($100).

(6) A student withdrawing from an institutional program, after attending more than sixty percent

     (60%) of the duration of the instructional program, is not entitled to a refund.
